The assassination attempt on the former president occurred a few minutes after the speech began in Butler, about 48 kilometres from Pittsburgh. Shortly after two gunshots were heard, the former president fell to the ground, clutching his ear, and was then covered by Secret Service agents protecting him.

With a bloodied face and ear, Trump stood up under his power, waving his fist at the crowd, to which the crowd responded by chanting "USA!". Moments after Trump's fall, a series of gunshots were heard from the authorities. Trump's spokesman, Steven Cheung, reported that the former president was unharmed.
